Duration: 13 Weeks Salary Range: $50 - $60 per hour Job Summary: We
are looking for a dedicated and compassionate Physical Therapist to
join our Home Health team in Portland, Oregon The ideal candidate
will be committed to providing high-quality therapy services in a
home setting and will thrive in a supportive environment focused on
patient care. This position involves traveling to patients' homes
and providing personalized physical therapy. Key Responsibilities:
Conduct comprehensive assessments and develop individualized
treatment plans for patients in their homes. Implement therapeutic
exercises and interventions to improve patients' mobility,
strength, and overall function. Educate patients and their families
on exercises, techniques, and home care strategies to support
recovery. Monitor patient progress and adjust treatment plans as
needed to achieve optimal outcomes. Maintain accurate and thorough
documentation of patient care activities in compliance with
regulatory standards. Collaborate with interdisciplinary teams to
coordinate care and meet patients’ needs. Travel to patients’ homes
within the designated service area; mileage will be reimbursed at
the government rate. Qualifications: Current state licensure as a
Physical Therapist in Oregan. Bachelor’s, Master’s, or Doctorate
degree in Physical Therapy from an accredited program. Previous
experience in home health preferred but not required. Strong
communication, organizational, and interpersonal skills. Ability to
work independently while maintaining a focus on high-quality
